Internal Medicine and its role in supporting evidence and high-value based practice

A new year and a new era are just around the corner, and the holiday season is in its full fervor. I hope all had an opportunity to reflect and celebrate the reasons to be grateful for – health, family, the privilege to serve our patients and being key stakeholders in our healthcare system. We are fortunate to have a crucial role not only to make complex diagnoses and initiate treatment and hope for our patients, but also to be leaders and have a voice to ensure all patients in the USA and the world receive evidence-based and high-value care. This entails a strong implementation of preventive medicine strategies, as well as appropriate utilization of resources for proper treatment of modifiable diseases.

Internal Medicine specialists and subspecialists are being exposed to fantastic discoveries – in the past month, wonderful treatments have been outlined in the most important medical specialty meetings in the world, and this entails better approaches for multiple diseases, from obesity and kidney disease, to cancer, heart failure, etc. We can't take all the knowledge and opportunities for granted. Investment in research is fundamental – it can't stop. Evidence of the best treatments to prevent devastating diseases must continue to be developed to ensure all patients achieve excellent outcomes and have their quality of life optimized. In addition to the enhancement of patients' life expectancy and quality of life, it also has a tremendous positive impact on our country economy with an extraordinary return on investment.

We must have a strong voice to support evidence. We must advocate for preventive care and recognize the impact it has had on our current health state. As Internal Medicine physicians we care for adults; but as a Pediatrician, I also care for children and young adults. It is my commitment to ensure that our children have all the necessary opportunities to develop themselves and achieve their potential of success and beyond. We have just survived a horrible pandemic with an impressive mortality and morbidity, and we have learned best practices to prevent and mitigate these devastating outcomes.

This is the season where we are seeing a steep rise in respiratory infections – most of them (at least in my practice) have been among frail patients who have not followed yet the recommendations for preventive immunizations. The point is not to create a divisive discourse. It is to realize that these frail patients can have devastating outcomes that could have been entirely preventable. It is our role to have the conversation with respect, understanding and with motivational interviewing strategies to help our community be aware of the available resources for preventive care, the evidence supporting its role, and have immediate access to these vaccinations without it creating a financial constraint on them.

So, let's all take a vow to continue being strong spokespersons for preventive care; let's learn new strategies to communicate efficiently and effectively with respect and consideration to our patients' beliefs, and to ensure that our education strategies are successful.

With this I want to transition to a different topic, yet with strong influence on the above. We are searching for a new chair of the ACP Ohio Healthcare and Public Policy aimed to focus on Ohio (we have a chair for the national affairs). This person will work along with the other co-chair and the OSMA to ensure we focus our healthcare and public policy efforts in alignment with the ܼˮ̳, but also with the best interest of our patients' best outcomes. This can involve engagement with other local medical organizations to support a more robust messaging based on the topic at glance. This past year we have been key stakeholders to work collaboratively with other organizations and would like to continue doing so. In addition, this leader will also be a key member to the trip to Washington, DC for the ACP Leadership Day. The visibility of the position and its relevance help to support the committee chair development within the leadership structure of the chapter.

A last thing to share, is that we are getting ready to create the final curriculum for our 2025 Annual Chapter meeting, and this involves the opportunity to be a speaker in the meeting. Please reply to Ms. Jan Wrassman's email asking for topics and interest to speak in our meeting. We want to provide an opportunity to all members to be a faculty and share their expertise. This past 2024 meeting was a true success, and part of this was the engagement of faculty who offered to be speakers and who did a spectacular job.

I want to express gratitude to all of you for your membership to our chapter. We are proud of the Internal Medicine workforce in Ohio – among the best in the world without any doubt. We want to make sure our programming and our activities are meaningful to you and your practice. And will continue to develop these always with the best interest in you. Thank you SO much for your support and engagement. We wish you and your families a very happy holiday season full of warmth, good spirits, excellent health and with great company. May 2025 come along with lots of energy and excellent outcomes for all.

With gratitude and respect,

Moises Auron, MD, FAAP, FACP, SFHM, FRCP, FRCPCH
