How does Patient Priorities Care work?

is an approach that involves aligning care among all of a patient鈥檚 clinicians with what matters most to that patient鈥攅specially older patients who have multiple chronic conditions for whom evidence-based medicine might not exist or be the best choice. It is about understanding each patient鈥檚 health outcome goals and health care preferences and ensuring that the care provided is in line with those goals. Patient Priorities Care resources address the What Matters 鈥淢鈥, the core element of the Age-Friendly Health Systems (AFHS) 4Ms Framework: what Matters, Medication, Mentation, and Mobility.
